DESCRIPTION:Siskiyou Community Health Center is offering sports physicals for high school students in the Grants Pass and Three Rivers School Districts. Physicals include a health history review\, physical examination\, and sports safety and education tips. Students under 15 years old must have a parent or guardian present.  \nPlease have a Sports Physical Form filled out prior to physical.
