Community Supports Program

Siskiyou Community Health Center provides the service of Community Health Workers for those experiencing instability in housing payments, food, or transportation.

A community health worker is a frontline public health worker who is a trusted and knowledgeable member of the community. This trusting relationship enables the worker to serve as a liaison between health/social services and community members to facilitate access to services.

If you or someone you love can use assistance in this area, please ask to speak to one of our Community Health Workers.
