Community Supports Program
Siskiyou Community Health Center provides the service of Community Health Workers for those experiencing instability in housing payments, food, or transportation.
A community health worker is a frontline public health worker who is a trusted and knowledgeable member of the community. This trusting relationship enables the worker to serve as a liaison between health/social services and community members to facilitate access to services.
