School-Based Health Centers

Care that meets students where they are.

Siskiyou Community Health Center offers permanent facilities within the schools that are staffed by a Family Nurse Practitioner, a Physician Associate, RNs, and receptionists. The centers are certified by the State of Oregon and meet specific standards of care. With these certifications, we can examine, diagnose, and treat school-age children. We work cooperatively with parents, coaches, counselors, classroom teachers, school principals, and their staff to ensure that the centers are an integral part of the life of the schools.

The purpose of our SBHC’s are to:

  • Attend to the unmet health care needs of children when they need it
  • Support students by providing a safe place to talk about sensitive issues such as depression, family problems, relationships, family planning, and substance abuse
  • Supports the schools by helping children stay in school and by identifying and addressing health problems that may interfere with the learning process
  • Support families by allowing parents to stay at work while attending to their child’s routine health care needs
  • Teach students to be better health care consumers

Virtual Visits are Now Available

Virtual visits are conducted by a live video chat on your tablet, smartphone or computer. We are committed to providing a safe, reliable service to students and faculty in our communities. If it is determined that a physical evaluation is necessary during your virtual visit, we will make arrangements for you to meet with a licensed clinician.

Grants Pass and School District 7
Virtual Visits are available to all students and staff in School District 7.
Virtual Visit hours: Monday – Friday from 8:00 am – 4 pm

Virtual Visits are available to all students and staff at North Valley High School and Fleming Middle School.
Virtual Visit hours: Monday – Thursday from 7:15 am – 4:15 pm
