MyChart puts helpful tools and health information in one convenient place.

View Test Results — See your laboratory and other test results as they become available.
Message Your Care Team — Send secure messages to your Siskiyou care team for non-urgent questions.
Access Your Health Records — Review visit notes, medications, allergies, immunizations, and other important health information.
Manage Appointments — View upcoming appointments and use available appointment-management features.
Check In Before Your Visit — Save time by reviewing and updating information before your appointment when online check-in is available.
Manage Billing — Review available billing information and sign up for paperless statements.
Connect With Virtual Care — Make accessing eligible virtual visits easier.

Getting Started With MyChart

Signing up is easy!

Step 1: Request Access

You can get started during your next visit to Siskiyou Community Health Center. You may also receive an activation link after your appointment or call our office to request one.

Step 2: Create Your Account

Follow the instructions in your activation link to set up your MyChart account. You’ll create your login credentials and complete the steps to protect your information.

Step 3: Start Exploring

Once your account is active, you can log in to MyChart from your computer or use the mobile app to access your health information on the go.

With the free MyChart mobile app, your health information is always close at hand.

Download the MyChart app from the Apple App Store or Google Play. After installing the app, follow the instructions to connect to your Siskiyou Community Health Center account.

Frequently Asked Questions

Is MyChart free?

Yes! MyChart is a free service available to Siskiyou Community Health Center patients.

Is my health information secure?

Yes. MyChart uses secure technology to help protect your personal health information. Your account is password-protected, and you can take additional steps to keep your information secure.

How do I sign up for MyChart?

You can sign up during a visit to Siskiyou, through an activation link sent after your appointment, or by calling our office to request an activation link.

What if I already have a MyChart account with another healthcare provider?

You can connect your Siskiyou MyChart account with other participating healthcare organizations, such as Asante, using MyChart Central. You will first need to establish your Siskiyou MyChart account.

Will my previous medical records appear in MyChart?

Medical records from before September 15, 2026, will not automatically appear in MyChart. Some older records may be added upon request.

If you need access to older records, please call 541-472-4777 for assistance.

Can I still access my old patient portal?

Yes. If you previously used Siskiyou’s InteliChart patient portal, you can continue viewing your older records there through October 20, 2027.

The old portal is read-only, so you will not be able to submit requests through it. It contains information from before September 15, 2026.

Can I use MyChart for urgent medical needs?

No. MyChart messages are intended for non-urgent communication and should not be used for medical emergencies or time-sensitive health concerns.

For a medical emergency, call 911. For urgent concerns, contact your clinic directly.
