How Do I Establish Care?

Thank you for your interest in establishing care at Siskiyou Community Health Center! The first step is to contact one of our Patient Services Representatives, either by calling us at 541-472-4777 or by visiting our Grants Pass Clinic or Cave Junction Clinic in-person. Our clinics are open Monday through Friday between the hours of 8:00 am and 6:00 pm.
When you call or visit, make sure to have the following information:
Insurance card(s), if applicable
The names and phone/fax numbers of any medical provider that you have seen in the last five years.
The Patient Services Representative will go over new patient information, including the New Patient Registration Packet, our Sliding Discount Program, and any additional questions you may have.
Before Your First Appointment, you will need to…
Fill out the New Patient Registration Application
This includes a Release of Information so that we may obtain your medical records for the last five years. These records are a necessary part of providing quality care and they are required prior to setting up an appointment. Requests for past records are faxed upon receipt of the signed Records Release and often take up to 30 days to receive.Submit the Slide Discount Application, if applicable
You will need to review the guidelines and proof of income requirements. The application and proof will need to be turned in at your first appointment.
Once the registration process is complete and past records are received, the Patient Services Representative will contact you to schedule an appointment.

Our dental services are designed to support your oral health at every stage of life, from routine cleanings and preventive care to more advanced treatments that restore and protect your smile. Our team focuses on providing comfortable, personalized care to help you feel at ease while addressing your unique needs. Whether you’re coming in for a checkup or a more complex procedure, we’re here to help you maintain a healthy, confident smile and support your overall well-being.
Our team offers comprehensive, individualized strategies designed to support the emotional, social, cognitive, and behavioral well-being of registered primary care patients within the clinic. We take a problem-focused, solutions-oriented approach, providing a diverse menu of services tailored to meet each patient’s unique needs and goals.
We has five School-Based Health Centers (SBHC) throughout Josephine County. Students perform better when they show up for class healthy and ready to learn. Our centers’ goals are to bring the doctor’s office to the students in a safe and familiar manner so they may avoid health-related absences, and to give them the support they need to succeed in the classroom.

Grow Well Program
Through Project Baby Check and Healthy Families, we work with parents during the critical early stages of their children’s lives, from conception to kindergarten. Our Outreach workers are parent educators certified through Parents as Teachers. We provide home visitations, monthly play groups, parenting classes, books and educational materials, nutrition and fitness classes, transportation services and much more.
Community Resources Program
We offer a variety of tailored services to support patients facing challenges with housing, food access, or transportation. Our Community Health Workers are trusted members of the community who help connect individuals and families with health and social services—ask our team if you or a loved one would like assistance.
