Sports Physicals Clinic @ GPHS

Siskiyou Community Health Center is offering sports physicals for high school students in the Grants Pass and Three Rivers School Districts. Physicals include a health history review, physical examination, and sports safety and education tips. Students under 15 years old must have a parent or guardian present.

Please have a Sports Physical Form filled out prior to physical. For more information or to download the form, please click here.

$10 cash or check fee is donated to school sports programs.
